raceface on bbs02 got shredded - postmortem help with images

I've been running the 36T raceface for many years. I have not been paying attention at this is what I saw today. You can see 2 of the 4 lugs have come off. I took off the chainring and replaced with stock today. See images below. Was the raceface not installed correctly? Did my chainline warp it? Where do I buy the plastic cover that has been ripped off under the chain ring?

Might need to check the chainring bolts more often.

If you've use Amazon, you can purchase the plastic cover there. The newer covers have seven screw holes.

If you riding the bike without the plastic cover now - well ... I'd not do that.

Also, your remaining ring bolts appear to have washers under the bolt heads - I'd think about fixing that up as well. There are various width ring bolt/nut pairs available, in case the originals were too long.
