O'course the one option that no one has mentioned to you is the Stoke Monkey. Direct drive or geared hub motor driving through your gear train via a tandem crank set and totally under your throttle control. For someone dealing with the combination of issues you've cited this would seem to be ideal!

I've used one of the Shimano Alfine tensioners with excellent results. They seem to have 2 designs that amount in different ways. Very high quality and reasonably priced.

If the LED is on, then the sensor is getting power. It has to blink when the magnet passes by. Maybe your magnet is too weak?.. Brilliant!
