Seems to be a hallsensor problem.
two phases on same level and one at a different assumes Block commutation. Normal for Startup.
Try to swap two hall wires.

Do you use a direct drive motor? It is well known, that the mosfets die with stock firmware, if you do regen with a nearly fully charged battery. If the BMS of the battery cuts the connection to the controller due to overvoltage protection during regen,you get very high voltage peaks on controller s...

This doesn't seem to be a reliable system. Broken springs, displaced magnets....
A good idea, but badly implemented.
Has anyone a proper working system? :-)

I would suggest to connect the torque-signal to X4, as the plug of the throttle may fit to the controller as it is...

Could X4 be used in place of the throttle wire? Or is that a digital input? You can use x4 for the torque signal, if you want to use the throttle in parallel. OK, first: you have to set the phase current limit to a much higher value than the battery current limit. e.g. max battery current = 15 amps --> max phase current = 30 amps.
