What is causing this freewheel noise on my A2B Metro?

I've had this A2B Metro for about a year now and last week I noticed this clicking/grinding noise coming from the freewheel. I took the rear wheel off and did my best to lubricate it etc but to no avail. It still makes this noise. What would be the best way to go about this?

*Note: After I put the bike back together I noticed I still had this washer/spacer looking thing left and for the life of me I cant remember where it was on the bike in order for me to reinstall it.
