Sevcon Gen 4 fault - DSP Encoder Fault

Hello,
I Have a Sevcon Gen 4 controller and a motenergy ME1115 PMAC motor that is constantly giving me DSP encoder faults.
Fault code: 0X52c1
Encoder input wire-off is detected.
Check encoder wiring.

The company I purchased the motor kit from says to just replace the encoder. I do this and it works ok for a few months (sometimes only a week) and then gives the same fault again. Well I’ve now replaced the encoder 6 or 7 times in the past 3 years and the controller once.

All wiring looks ok but today I noticed the voltage coming from the controller to the encoder reads at 3.1V instead of 5V. Can this value fluctuate? I don’t think the previous encoders would work if the value is always 3.1V.
The programmed parameter is set for 5V.
Anyone have any ideas as to why this keeps happening?
