The benefits of an extended warranty are secondary, compared to other factors which will make me choose to purchase an ebike over another. Still, my question is: What manufacturer provides the best extended warranty value? Why is this important? For the premium you pay, but it’s also an indicator of the manufacturer’s trust in his product. Even if you choose not to get the extended warranty, it would tell that should be what I would expect to pay myself for problems after the base warranty expires.
2 manufacturers offering extended warranties that come to mind:

What’s interesting here is that the batteries (which seem to be the principal concern of most manufacturers) are already covered for 3 years (or 30,000 miles) in the base warranty, so the benefits are for other areas.

375 $US for adding an extra year to the base warranty. 725 $US for adding an extra 2 years.
Base 1 year warranty covers all components (including the battery).
Any other ebike manufacturer offering extended warranties? Most other manufacturers typically provide 1 year on all components, except the battery which is covered for 3 months. And there is no possible extended warranty.
It all comes down to what risk you are willing to assume when purchasing an item (toaster, TV, car). Myself, I pass on extended warranties when something has a wide distribution and is less than 1000 $US, but the more expensive an item is above that price or the smaller the production level, the more I want to avoid risk of costly repairs by purchasing an extended warranty.
I had 2 Ezee ebikes for the last 9 months, with their batteries out of warranty, and (sound of knocking on wood) no problem.
