Why do Hall Effect Sensors break? (UPDATE, AGAIN w new 5404)

I wish he would have said what fixed it... I'm having this exact same issue right now. Have already tried a few of these suggestions, now going to try replacing the hall wires and checking inside the controller to see if anything looks off about the hall wire connections in there. Will also look for spikes on the vcc line.
 
Buy some new hall wires, might as well do the phase wires too. Buy new hall effect sensors.

Use a Sensorless Controller.
If you still want hall effect sensor controller, then buy a new one.
 
I had all 3 Hall sensors fail in a 5303 after the motor worked fine on the bench. It turned out a red(+) wire was shorted to the axle.
New Halls and fix the short was the solution. I had tested all 5 hall lead wisre combinations for shorts but didn't think to check for a short to the motor. I hope your solution is as simple.
