I've never heard of a 18650 explode during normal use. It can happen when there is no BMS and one or more cells are unbalanced and get charged to over 4.2v, in that case most cells would probably explode. Every cheap powertool from stores like Lidl & Aldi and whatever all use garbage Chinese cells. I have over 50 powertool packs in my house because one cell shorts internally and drags the other down with it. I always find a few einhell and active x batteries at my local scrap yard.
The reason they don't explode is because the BMS tells the charger not to charge when one cell row is too low or too high etc.
A good powertool BMS will have a sense wire go to each cell row.
