Not exactly US but close is naked cycles in Canada. (Quadra island). They do great award winning customer builds.

There is a custom builder over in Napa named Curtis Inglis that makes some of the highest quality frames I have ever seen. All of his Retrotec models have beautiful, flowing curves..

I would dig mounting a mid-drive, water bottle battery and controller in the seat bag of one of these set-up as a Hardtail ..mmmmm good..
